Partnership Overview

Brook.ai and SRHO (The National Association) have partnered to bring AI-enabled continuous care infrastructure to more than 275 hospitals nationwide. The partnership represents a significant scale deployment of continuous care AI, moving from individual hospital pilots to a national network implementation.

Continuous Care Model

Brook.ai's platform provides AI-enabled continuous care — ongoing monitoring and intervention between clinical encounters. The system tracks patient-reported outcomes, physiological data, and behavioral signals to identify deterioration, non-adherence, or care gaps between scheduled appointments. This model is particularly relevant for chronic disease management where most patient time is spent outside clinical settings.

Scale Implications

Deployment across 275+ hospitals creates a large dataset for model improvement and population health insights. It also raises questions about standardization: continuous care AI deployed at this scale must perform consistently across diverse patient populations, care settings, and clinical contexts. Governance frameworks for a network-wide AI deployment of this size are a significant implementation challenge.
